We do our best to gather as much information on your individual plan prior to your appointment, however it is ultimately the patients responsibility to know and understand the policies and benefits of your plan. We understand insurance can be tricky and we are happy to answer any questions you may have.

CANCELLATION POLICY

In the event you are unable to keep a scheduled appointment, please advise us as soon as possible. This allows us to utilize our schedule to the best of our abilities. There is a cancellation fee for any appointment cancelled within 24 hours of the scheduled appointment.
